PREVENTION SPECIALIST

JOB SUMMARY:

Provide preventative education and counsel to at-risk populations to protect their health, safety, and welfare. Maintain a caseload of 25 children assessing them for risk of abuse and neglect and linking them with needed services. Perform home visits.

EDUCATION:

Bachelor's Degree from an accredited college or university in human services or a related field

EXPERIENCE:

Six months experience in counseling/case management preferred.

PRIMARY JOB FUNCTIONS:
  • Assess families for risk of abuse and neglect. Complete assessments - initial/90 day.
  • Link at-risk families with services.
  • Conducts in-home services; in-home parenting, behavior modification and money management/Family Treasure/life skills/case management,
  • Builds relationships with referral sources , Department of Children and Families and service providers.
  • Provide regular contact with families by performing home visits.
  • Maintain required files on each client.
  • Assist in obtaining referrals.
  • Staff cases with supervisor.
  • Visit children within 30 day period as indicated on family assessment.
  • Will complete errors on file review by next month's file review.
  • Complete other duties as assigned.
  • Provide written court reports if required.
  • Attend court proceedings relating to case, including providing court testimony if necessary.

Clear a local background check from Duval County and the County in which you reside.

Clear a drug screen.

Provide a copy of your educational attainment on your first day of employment.

Clear a Motor Vehicles Records check.

Provide proof auto insurance and maintain a minimum personal auto bodily injury liability insurance.
